Advance Care Planning lets your caregivers, doctors, or family members know your end-of-life preferences in advance. Attend this presentation by Jan Wilson of End of Life Options New Mexico to get tips for how to talk about end of life matters, what documents you should include in your plan and how to share your plan with others to ensure you get the care you want at the end of your life.
